My current H4 will expire on 12/8/09, can I apply for extension by myself next Monday 12/7/09?
-Yes, but it is a litlle late. You should file the extension early (at least two weeks before your I-94 expires) Anyway, I hope it is still OK.

If I can do it by myself, how many documents should I to USCIS? Please give me a list.
-You need to file I-539 and provide the document mentioned in Form I-539 instructions.
Here are documents you may need to provide with the I-539:
Copy of Your spouse H1 Approval Notice and his last three month paycheck stubs, copies of your clear passport copies of biographics and visa pages, marriage proof (not needed but include it), copy on unexpired I-94 and any other documents required by in the I-539 instructions.

I am living in Chicago, Illinois, which service center should I send the application to?
