1. with AP approved before you leave US, you can use AP to return if H1 visa fails.
2. If your I-485 is approved, you can no longer get nonimmigrant visa. Or if consulate giving you visa by mistake, the CBP should act accordingly (let you in as LPR or using AP, but not in H1 status).
3. If consulate gives her H4 visa by mistake, CBP should act accordingly. (let her in not as admitted, but deferred inspection) the chance of denied entry is slim, if she has a valid H4 visa.
