Either you were not in US when I-485 was being filed, or you left US w/o AP after I-485 filing and re-entered in B visa. Your I-485 is not approvable.

You may re-file I-485 any time, regardless your I-94 expires or not. You may leave it blank.

You may request an I-94.

Better not to leave US, even with an AP, if I-485 is filed beyond I-94 expiration.
