I think this lawyer misunderstood your situation. If your I-485 and the I-693 have been filed to USCIS, this I-693 can be valid > 1 year until your I-1485 is approved or denied. But in your situation, you have not filed I-485 along with I-693 yet. So, your physical exam record is only valid for one year.

U.S. Citizenship and Immigration Services (US CIS) issued an Interoffice Memorandum to Regional Directors, Service Center Directors, Field Office Directors and to the National Benefits Center Director informing that the validity period for most medical exam certifications will be extended beyond the validity period of one year and up to the time of the adjudication of the adjustment of status application. This new policy was implemented to address the problem of backlogs of certain types of applications for adjustment of status (I-485 Applications) where adjudication is now taking longer than one year. Only those applications that include a Class A or Class B medical condition certification will not be extended beyond the one-year period of validity.


All applicants for adjustment of status are required to file, along with the Form I-485, G-325A, Biographical data, Form I-864, Affidavit of Support, supporting documentation to evidence eligibility (depending on the type of case), and, a Form I-693, certified medical from a US CIS designated physician. Those medical exam certifications are generally valid for one (1) year.

In recognition that processing times for those applications for adjustment of status at the various US CIS Service Centers, Field Offices and National Benefits Center are taking longer than one year to process or adjudicate, last January US CIS sought a recommendation from the Centers for Disease Control and Prevention (CDC) to determine whether that validity period could be extended.
