I would like to request a change of status from my F-1 Student status to that of H-4, dependent of an H1B (temporary worker).

My Spouse, has already been granted H1B status to the period from MM/DD/2003 until MM/DD/2006. Her receipt # is SRC-03-***-****, and I-94 # is *******.

I have enclosed the following documents for your consideration:
• Form I-539
• $140 Application Fee
• A copy of the employee’s (my spouse’s) Form I-94 and approval notice showing that she has already been granted status to the period requested on my application
• Marriage Certificate in Chinese and its translation to English showing my spouse relationship to ****. We have been married since MM DD, YYYY
• Letter from Ms. ****, International Student Advisor of *** College, verifying my legal status as a Full time student during fall 2002 to spring 2003 semesters
• Transcripts of courses I took in fall 2002 and spring 2003 semesters
• Copies of I-20 issued by *** College (one is with SEVIS mark, one was original not in SEVIS mark)
• Copy of My Form I-94 and approval notice for a Full time student
• Copy of My passport and all F-2 visa stamps
• Copies of MY spouse’s I-20 from University A and University B
• Copies of My Spouse’s Diploma from University A and University B
• Copies of Bank Statement for US with Bank of America for months of May, June, and July 2003
• Copy of My Spouse's passport and all F-1 visa stamps
