请问PJANG等大侠：什么是Golden Sheet?

收到USCIS的RFE，要我们补交Medical Examination Report。我老婆在交485时怀孕，没有做TB，这个要求正常。不过在邮件里有这么一段话：

This office is unable to complete the processing of your application without further information. Please read and comply with the request below, then submit the evidence to the above the address. Include a copy of this letter and place the attached golden sheet on top of your documents.

我就收到一封信，不知道Golden Sheet是啥？请指教。
