"The documentation submitted is not sufficient to warrant favorable cosideration of your petition/application".

You appear to have neglected to submit any photo identity with your adjustment application.
YOu adjustment application much be accompanied by documentation establishing your identity. Therefore, submit a clear photocopy of a current photo identity document showing your picture, name and date of birth. Acceptable documents include passports, driver's licenses, state of national identification cards, or other identification issued by a government agency.
