3. Bank Statement /银行存款证明
4. Photocopy of passport, I-94 and F-1 visa /护照、I-94表格、F-1签证复印件
5. Photocopy of social security card, driver’s license, and medical insurance card/ 社会安全号（SSN）、驾照、医疗保险卡复印件
6. Photocopy of W-2 and pay-stub (TA/RA) /W-2税表和工资单
