AOS status, 2nd 485 questions.--急问

Teacher Pjiang and other teachers:

About my 2nd 485 application, I am AOS status (used AP), teacher Pjiang told me to check the latest I-94 card, it is "paroled until 11 Jul 2009".

So, the AOS expire on "11 Jul 2009"? or leave it blank. (485 form part 1).

Another question, 485 form part 3, status is AOS, so, all the "nonimmigrant visa number", "consulate where visa was issued", "date visa was issued" leave blank?
