I-864 and other questions on GC application

Hi Jiling and other teachers,

I have couple of more questions:

1) Does I-864 need to be filled out in CAPITAL LETTERS throughout the form?

2)If filing I-130 and I-485 concurrently, is the following address is the correct address when sending by priority mail from post office?

Attn: FBAS
131 South Dearborn-3rd Floor
Chicago, IL 60603-5517
