Power of Attonery need to be filled and signed by the parents.

then when I returned the Power of Attoney to the PLANO ISD,the lady asked me to fill out this 监护 form so the kid can go to school under my supervision. I am not sure this form is related to the ISD or the general 监护form.

PS. Vaccinations from China is not enough, they asked the kid to give more Shot from the healthy center.
