When I did mine before, I went to the international student center's website, downloaded a request form. This form listed all the required materials, for example financial proof (bank account statement --you have to go to the bank ask for this, or RA or TA appointment letter).

I remember my advisor had to sign the form -- explained the reason why I had to extend it, and the expected graduation date.
