urgent question about I485

LD found his previous company didnt revoke 140, and we are trying to relink
his current 140 to his prevoius 140 to get the earlier PD.

In this case, if we want to rush to get 485 in before end of month, is it
still possible(his first PD is early 2006). If we can't get time to do
medical check up, would it be a big issue if we submit later?
