(1)Yes. The doctor used a new I-693 (Rev 04/02/08) to report our TB test results.
(2)Yes. The doctor's signatures are original.

It seems that the USCIS sent out the 2nd RFE (Aug 27th) before they received our response (Sep 2, indicated in the online status of my wife's case).

Strangely, in the 2nd RFE the USCIS asks for the I-693 supplemental form, which was sent to them together with I-485. This supplemental form is a part of new I-693 form (at least is the case in Rev. 04/02/08). Do not understand why they asked separately.

Luckily, we obtained envelopes with new I-693 form and the supplemental I-693 form enclosed from my civil surgeon.

Can I send it out by myself, not through the attorney?
