J1 waive, 请知情的热心人帮忙.

我的情况是这样:

去年从J1 转成 O-1 (没有waive, 回国重新签证入的境),并在去年7月递交EB1-b 140,
已在上月批准. 我现在需要要办理waive , 请问我的statement of reseason 该怎么
写?
