re-ask, how to address the weird I-485 RFE

USCIS ask to provide evidence of "permanent position" for my NIW based I-485. Can research fellow satisfy "permanent position"?

2) I provided my medical examination in 2005 when I submitted my I-485, now they asked to provide medical examination again. My wife is pregnant and can not do the examination now. What should I do?

I just received delayed USCIS letter and I have only 3 weeks left to answer their requirement. Please help!
