your doctor should have a copy of your original medical forms, call him and fax him the RFE so he can check it and give you a new signed form, worst case is you pay some more money to him. I got medical RFE also and CIS don't need the old form mailed back so just send back the new form signed by your doctor. It shouldn't be a big problem. Good Luck!
