I would not suggest you to file untimely application, only because of worries on address issue.
1) You do have 60 days grace period, upon expiration of OPT, to file application with USCIS. However, I strongly recommend you not to do so. Always file timely application, at your every possible.
2) I would file timely application at the current address, and file AR-11 whenever the address is changed.
3) I don't think there are extra fields for USCIS to input your extra information provided on additional paper, besides the application form itself.
4) I will never worry about FP/Interview issue, as they may be rescheduled at a different time and a different location.
