1. Yes, you need to apply for visa to visit Canada.

2. You can go to consulate general of Canada in U.S. depending on where you are. I applied for visa in New York City yesterday. Not very hard to get visa.

3. Use AP to come back to U.S. You can use it as many times as you can before it expires.
