You may apply extension within a month. Under the regulations, if an alien's extension application has been pending more than 90 days, he or she can continue to work for the same employer for 270 days.
The cost varies depending what type of H-1B (exempt or non-exempt). Generally you have to pay filing fee, $500 fraud fee, $1000 training fee plus attorney's fee.
