hi, a question regarding physical exam result

I took an exam but I just found out that on the paper of the physical exam result my doctor did not put remarks on the following part:

3. Physical or mental disorders with associated harmful behavior
4. Drug Abuse / Drug Addition.

Is that ok for my greencard application? or Should I require my doctor to put their remarks on the above two part? Thanks for your help.
