Thanks for suggestions! I think the refs were issued by one IO but we received in 3 separate envelopes.

I made an appointment with a new civil surgeon because my previous Dr. was retired 2-3 months ago and his office was closed. I didn't see the new Dr. yet, I will update my situation here after Dr. appointment.
