Citizenship RFE. Please help.

Hi,
My friend received an RFE for citizenship. She received this letter and the second page is called "Record of sworn statement in affidavit form". The second line is "Executed at ___________" "Date _______".
I assume you fill the first blank with your home address, is that right?
