1. It doesn't have to be your immediate boss, it would be better be. You can have someone in the HR department of your company can write a letter using your company letter head. You should leave contact info so cic can do background checking.

Answer to your third question is Yes.

For the first question, you should phone cic using their free 1 800 number.
