Thank you for your help! I changed the doctor because moved to diff city. The current doctor doesn't have anything from my original I693 form.

Did your doctor fill in the new I693 form according to the old record + the new TB test result OR he just attached a copy of the old I693 with the new I693 (only has TB test on it)? Thank you!
