Please help me on NIW RFE, Thanksssssss

一年前file 140 时研究方向是diabetes，现在收到RFE,但是研究方向是heart disease。请问新准备的recommendation letter 该怎么写。是只写diabetes，还是diabetes和heart disease都写。Thanks.
