1. your I-20 doesn't need to be included in the OPT application.
2. DMV maybe a problem. You have F-2 I-94 that is D/S, and also F1's I-20 showing your name as dependent and F1's EAD proving OPT approval. That is good enough for USCIS but DMV people may or may not know that.
