H1 premium申请问题？

公司已经file了我的H1申请，可是不给办加快，准备自己办加快。I-907表有些问题还不
是很清楚，希望有经验的人给指点一下。

在part1中
Name of Company Contact---我还是公司负责我h1申请的人？

在part2中，
Form number of related petition or application.
I129的receipt number?

Classification type being requested
H1B ??

Petitioner on the relating case---我？
Beneficiary on the relating case---我还是公司？

另外，我没有I-797（receipt）copy，会不会影响申请？谢谢！
