I-140 REF help

The attorney emailed me the 2-page REF after I requested. The 1st page says: please read the attachment for details, which I believe it is on the 2nd page. But the 2nd page seems not right: no start, no end, as shown following.

BTW, before receiving the email, we talked on the phone, he said: it is not that bad. Just something like providing the proof that your company hires more than three employees, etc. But I could not find this on the 2nd page, either.

I did email him again to confirm the content. He said that's all he got. Is the attorney hiding some content of the 2nd page from me? What should I do? The refer due date is in less a month from now on. I feel very nervous!
