I believe XBT will give you accurate answers. Prior to her answer, please consider some of these:

1. How much is the filing fee if all the applications are filed together? Is the following calculation correct? Should we write only 1 check or separate checks?

Certainly when you pay $1010 for 485, that includes EAD and AP filing fees. One of the lawyers seems too old-fashioned and keeps no updates on USCIS new fee policy announced in June.
Recommend to write separate checks for 130 and 485.

2. How long can the foreigner begin to apply for citizenship?

For LPR married to a citizen (through family-based GC, not EB GC first then married to a citizen), s/he will be able to apply for naturalization at 3 year timeline, on condition that s/he has been physically resided with the citizen spouse for 3 yr, plus the citizen spouse should qualify for holding at least 3 yr citizenship.


3. Has anyone been to USCIS local office for filing questions?

Refer to your question. Depends on the attitude of officer.


4. Does the "copy" in all the applications mean "photocopy"? There can be official copy for birth certificate and marriage certificate. Should we official copy or just photocopy?

Just regular xenocopy of a notarized certificate, not necessary to provide color copy. You can certify the copy if you want perfection.

5. can part of the medical exam expense be covered by insurance?

yes, some immunization shots such as DT, or X-ray expense can be waived by certain insurance providers.

6. Does family-based petition application have premium processing?

Don't know 130, Not 485 at least.
