Where to get FBI finger print card? Thanks!

For Canadian immigration purpose: How to get a FBI finger Print Card? My
local police office has finger printing service but they do not have the
card, local FBI office said they can not give out blank FBI finger print card.

Can I use the form download online? Does FBI and Canadian Immigration office accept the download form? Thanks a million!
