Pjiang 老师，pls help for I-765 form

I forgot to provide an answer on I-765 form # 2 for the other names used, now USCIS need me to provide a copy of a notarited statemen of the other name used, buti don't have, i only use one name.

So what can i do right now? do you think i just fill out another I-765 to USCIS.
