H1B transfer DIY - 问题求教

刚刚拿到我的H1B才两个月，公司就从分裂了。我跟着分出来的公司，又得开始折腾H1B transfer。上一次是请了律师，我留了一套申请的材料。这次，我决定自己办。东西弄得差不多了，就是有几个问题拿不准，向各位大虾请教：（我是从NONPROFIT 转NONPROFIT）

Question 2: Shall I check “e – Change of employer” only? Or I shall check both “a – New employment” and “e- Change of employer”?

Question 3: shall I put my current H1B receipt number in the box?

Question 4: do I need to fill in this box with my previous application receipt #? I am not applying to change and/or extend my status. I am just applying for a H1B transfer. Does that count as “status change”?

Question 5: Which box I shall check, “b” or “d”? I feel “d – Amend the stay of the person(s) since they now hold this status” is proper because I am holding H1B.
