WOM question-proof of service certified mail need help

I sent the complaints to defendents, attoney general and attoney in the district by certified mails. I got the green color cards from some of them. I want to give the cards to court, but she told me I need write proof of service(no sample) and attach the card to it.

Please help me and give me the sample if you have. I wonder if I need submit the original cards?

I do not how they count the time (60 days), from the day defendent got the summons or the day I filed the all certified green card to them?
