He/She recommend,& his/her supervisor approve.

Different suitations may cause delay: If background check not yet, nobody of CIS can't approve the case;
If the IO doubt your marriage is real one, he/she will
freeze the case until second interview ( usually need to wait 1.5 - 2 years )
