Send another application form with 2 photos and the original AP you got. Write a cover letter to explain the mistake in AP. If the mistake is not taken by you, you don't need to pay the fee again.

My son got his AP with my date of birth there. I sent the AP back with the application form. We got the new corrected one in about half month.
