Urgent! please help, AP mistake

We are July 2nd filer, I am the main applicant with approved I-140 at TSC.

We got EAD cards, both OK.
We got FP notice, both OK.

Today we received the approved AP from my lawyer, it is only for my hu*****and. But it has my photo on it with my hu*****and's information.

How did this happen? What should we do!

Please help, many many thanks!!!
