B-2 with new passport

My mother is going to apply for B-2 visa in Guangzhou again on october 24 with new passport. Does any one know if she need incicate that she has been rejected the B-2 application before? Does the visa officer check the record by name or by passport number or both? Or maybe we had better write down the B-2 application was rejected before?
