1） local office现在还可以办时的AP吗？（她已经去了三次了，都没有拿到卡，officer一定要她等中心寄出的卡）

They should give her. My wife didn't get the AP because they said it's not your parents in critical situations. If possible, she can go to local congressman for help. In general, they'd like do this kind of thing. Also she can try more times to have more chances to meet a nice IO.

2） 如果可以的话，需要哪些强有力的证据。
the doctor's diagnostic is best evidence, she can ask other people, like her colleges, boss, congressman to, write a letter to confirm it. Maybe it's helpful. they double it.

3）她是否可以不拿AP卡，直接回国，在国内签证后用H1B入境吗？这样的话会有什么影响吗？以她现在的情况，如果用H1B签证入境（在485pending期间），是否等于自动放弃485？

Yes, She can go back for applying H1 in China. My wife did it last year in same situation. My wife re-newed her H1-b too but haven't got the notice. So she brought both new H1-1 or old H1 doc to apply H1-b. The officer finally accpeted the old one and gave her three months visa until expiration. Actually it's good because the old one is approved, her status continues, so shouldn't impact 485, (our lawyer said). what she needed was only a stamp. no any other change.

Suggestions: 1) call for visa appointment as early as possible. giv e herself more time for the visa. they always change the procedure. We were in little trouble last year. 2) brought all doc with her for the h1 visa, old and new.
