i-129 form questions, plz help

I-129 forms questions:

Part 2:

#2: for h1 transfer, i should select "change of employer", right?
#3: receipt number: i have two receipt numbers, because i applied h1 three years ago and did h1 transfer one year ago. should i give both of them or the latest one?

Part 4:

should i select no for all the items 3~10? i looked my previous i-129 forms, the first form all no, but the second form no choice was picked.
