According the letter from USCIS, the civil surgeon should give me a chest X-ray if my skin test is 5mm or bigger. "The civil surgeon should also refer you to your local health department for further evaluation to determine whether you have clinically active TB and if so, whether it is infectious. The Form I-693 must be properly endorsed by the civil surgeon to reflect all of the following: 1) The date you were referred to the local health department for further evaluation, 2) The name, address and telephone number of the local health department, 3) The results of the X-ray and any other diagnostic tests for TB, and 4) The basis upon which you have been medically cleared for adjustment of status.
