since not 100% patients with positive TB can be detected with X-ray.
is there other concerns besides the size of your induration, such as wasting? are you a skinny person? has she listened to your lung? she would subject her career to risk if she suspected you could have active TB but failed to refer you for further examinations.

most likely, you will need a sputum test, which includes a smear test and a culture test. the smear test is to stain the bacteria, if there is any, and look for them under microscope, so this test is fast. if your smear result is negative, then they will take your sputum to do culture test. the culture test is to look for colonies of tuberculosis, it will take 6 weeks to confirm a negative result.

I think you should go to your physician, if your own physician can provide a letter to this doctor confirming negative TB, you'll be able to get the form filled.

without the form, you are wasting money filing 485.
