I-485 refile

我刚收到移民局要我重新递交个别文件的信。要我递交：

Submit the following evidence to verify the birth and parentage:
1。 A photocopy of the Household or Village Registry reflecting the entry of me.
2. A photocopy of the booklet issued by the province's health department reflecting my name, gender, date of birth, and both parents' name.

我想请教：第一个问题是不是要递交一张类似全家福的照片？关于第二个问题，本人不记得有过出生证 (生于1970)。是不是要户口本？可是我在国内时是集体户口(毕业后在北京工作)，户口本不在我手里。我也不清楚自己的户口本在哪里。
