请问I-765表第11项，关于以前得到OPT

以前申请过OPT并得到EAD 卡.
问题是我没有保持批准的信件,也不知道是那家USCIS Office,何时批准的.
我根据那个EAD卡上的Receipt Number",在USCIS网站上查case status seach. 结果显示说"On September 28,2005, we mailed you a notice that we approved this I765 Applicaiton for employment Authorizaiton. .......

把这个打印出来可以吗? 是不是可以填September 28,2005为批准date?
