Problem of AR-11, Help!!!

In the form, it asks for "copy number from alien card". What alien card is that? My H1-B card or I-94 No.?

I came as F-2, changed F-1, H-1 and just changed H-4. Should I put Visitor or Student? I went to Canada for F-1

4. Date of entry into U.S. -- should it be my last date of entry, or first entry date?

We have moved several times and I never done the form yet. Should I insert a letter? What should I say? Apologize or else.

Please help, Xiaobaitu or any friend. Thanks a lot
