Nee d Help - How to apply for fast AP from local office (Atlanta

My dad has heart problem, it is not allowed for long time in the air. I have not seen them for five years... I want to visit local office to see if I can get a AP faster... What kind of of docuements need? Do I need to translate the doctor's proof in English.. notorized? Thank you for your help!
