according to USCIS policy, if you are NIW, need COPY of 140 receipt or aaproved; else if PERM or RIR, need LC COPY and USCIS online pending COPY. So I guess why your case was returned. You should inlcude the COPY of 140 notice not only the 140 receipt number!! But you can mail again so you are so lucky, hehe.
