Was it returned by carrier?
- package unopened.

Was it rejected by USCIS?
- package opened and re-sealed in USCCIS envelope, addressing to you

Was there a notice enclosed?
- normally a blue sheet to explain why it is rejected.

Last, did you mail I-140/I-485 together and only I-485 came back?
