哪里下载ETA 750B表啊? 我在DEPART. OF LABOR找到这个

看着不太对啊,查旧贴大家在说三页啊四页啊,这个是三页,可是最后一页很可笑的是什么"PRIVACY ACT STATEMENT"

要晕倒了,刚刚才知道需要这个表:( 更要晕倒了,找不到这个表:(
