Help- how to apply for AP in a local office? website see insid

How can I get AP approved in local office, waht kind of documents needed? Does anyone know website or contact phone #? I have not seen my parent for 5 years, and they can not come over to see me due to health issue. Not a very emergency issue, just Dad has hearth problem, he can not stay in the air for that long time... Anyway, can anyone help me??? Thanks!
